The Care Coordinator plays a crucial role in connecting individuals with chronic behavioral and medical conditions to essential health services. Key Responsibilities
Linking adults and children with chronic behavioral health and medical conditions to necessary services
Developing person-centered care plans with members and their families
Providing outreach services and responding to member inquiries
Tracking and arranging appointments and referrals
Completing required documentation and sharing knowledge with team members
Required Qualifications
Bachelor's degree in social work, psychology, nursing, or related field
Two years of experience providing direct services or case management to mentally disabled or chronically ill individuals
Ability to engage with clients and provide culturally competent services
Strong verbal and written communication skills
Experience working in interdisciplinary teams
Preferred Qualifications
Fluency in a second language such as Spanish, Russian, or Creole
Experience in care management or care coordination
