Job Description

Key Responsibilities
Supply appropriate interventions to ensure client care
Develop action plans to identify and utilize existing resources
Conduct routine community contacts
Locate and utilize appropriate community resources for clients
Evaluate living conditions of clients through home visits
Individualize client treatment and facilitate referrals
Provide individual/group counseling based on client need
Perform community outreach and establish linkages with relevant organizations
Provide case management to clients
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Spanish
Bilingual Haitian-Creole
