Job Description
Join The Jewish Board as a Case Associate, where you'll play a pivotal role in supporting clients to lead independent lives. This position involves client care planning, community resource utilization, and providing essential case management services.
Key Responsibilities
Function as a case planner and provide appropriate interventions for client care
Develop action plans utilizing existing and new resources for clients
Conduct routine community contacts and evaluations of living conditions
Run groups under the supervision of a social worker
Provide case management and individual/group counseling
Perform community outreach and establish linkages with other organizations
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
