Position Summary:

VitalHire is seeking a skilled and compassionate PT - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Rye, NH. Key Responsibilities:

• Provide direct physical therapy services to patients in skilled nursing facility setting
• Develop and implement individualized treatment plans based on patient assessments
• Conduct initial evaluations and ongoing assessments of patient progress
• Document patient care and progress in accordance with facility and regulatory requirements
• Collaborate with interdisciplinary team members including physicians, nurses, and other therapists
• Educate patients and families on treatment goals and home exercise programs
• Maintain accurate and timely documentation of all patient interactions
• Participate in care plan meetings and discharge planning
• Ensure compliance with state and federal regulations governing skilled nursing facilities
• Maintain current state physical therapy license and required continuing education
• Adhere to facility policies and procedures
• Work scheduled shift hours from 08:00 to 16:30

Job Requirements:

• Active, unencumbered license in NH
• Minimum of 1-7 years of relevant experience, depending on specialty
• Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
• Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

• Current Resume listing detailed work history (MM/YYYY format)
• Two current clinical references
• State-required professional license
• Physicial exam (completed within the past 12 months)
• TB skin test or Quantiferon Gold (within the past 12 months)
• Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
• Current Tdap and Flu vaccinations
• Must pass background check and drug screening
