Job Title: Home Health Physical Therapist Assistant (PTA) 

Position Summary: 

The Home Health Physical Therapist Assistant provides physical therapy services to patients in their homes under the supervision and direction of a licensed Physical Therapist (PT). The PTA implements treatment plans designed to restore mobility, improve strength and balance, relieve pain, and promote independence in daily activities. The PTA collaborates with the interdisciplinary home health team to ensure quality and continuity of care. 

 

Key Responsibilities: 

  • Implement physical therapy treatment plans established by the supervising Physical Therapist. 

  • Provide hands-on therapy interventions including therapeutic exercises, gait training, balance and coordination activities, and functional mobility training. 

  • Educate patients, family members, and caregivers on home exercise programs, proper body mechanics, and use of assistive devices. 

  • Observe and report patient progress, responses to treatment, and any changes in condition to the supervising PT and other care team members. 

  • Assist in reassessing patient goals and updating the plan of care as directed by the PT. 

  • Accurately document all patient care activities, visits, and observations in compliance with agency, state, and federal guidelines. 

  • Maintain effective communication and collaboration with the supervising PT, nurses, occupational therapists, and other members of the home health team. 

  • Participate in case conferences, team meetings, and continuing education as required. 

  • Ensure compliance with all agency policies, safety standards, and regulatory requirements. 

 

Qualifications: 

  • Education: Graduate of an accredited Physical Therapist Assistant program. 

  • Licensure: Current, active state license as a Physical Therapist Assistant. 

  • Experience: 

  • Minimum of one year of clinical experience (home health experience preferred). 

  • Familiarity with electronic medical record (EMR) systems preferred. 

  • Certifications: Current CPR/BLS certification. 

  • Skills: Excellent communication, interpersonal, and organizational skills; ability to work independently and manage time effectively. 

  • Other Requirements: 

  • Reliable transportation, valid driver’s license, and current auto insurance. 

  • Ability to travel between patient homes within assigned service area. 

 

Physical Requirements: 

  • Ability to lift and transfer patients safely. 

  • Frequent standing, walking, bending, and reaching during patient care activities. 

  • Ability to work in various home environments and conditions. 

 

Work Environment: 

  • Home-based patient care settings. 

  • Regular collaboration with clinical team members via in-person and electronic communication.
