Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You'll be responsible for developing care plans, conducting home visits, and connecting clients with vital community resources while enjoying comprehensive benefits and a supportive work environment.
Key Responsibilities
Supply appropriate interventions to ensure client care
Develop action plans to utilize existing resources for client care
Evaluate living conditions of clients through home visits
Provide structure and guidance in independent living skills
Run groups under the supervision of a social worker
Evaluate clients for individualized treatment programs
Facilitate referrals to treatment or service providers
Provide individual/group counseling
Perform community outreach and establish linkages with other helpful organizations
Maintain necessary documentation and progress notes
Required Qualifications
BA Degree
Experience in a community or social service group
Five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual in Spanish
Bilingual in Haitian-Creole
