Job Description

Join UnitedHealthcare as a Field Tribal Community Health Representative, where you'll engage with Tribal members to enhance their health and well-being. This full-time role involves working closely with communities to provide essential health services, promoting self-advocacy, and addressing unique health challenges faced by Tribal populations.

Key Responsibilities

  • Engage Tribal members to improve their health, including taking vital signs and transporting them to appointments
  • Advocate for members and promote self-advocacy
  • Provide education on community resources and benefits
  • Foster cultural competence and share knowledge with community members
  • Work with care coordinators to develop person-centered care plans
  • Support members in improving their wellbeing and accessing healthcare
  • Conduct post-discharge activities and assist in connecting members to social services
  • Other duties as assigned

Required Qualifications

  • High School Diploma/GED (or higher)
  • 2+ years of experience living or working in a Tribal Community, preferably in Idaho
  • Intermediate proficiency in PC navigation for applications, emails, and data entry
  • Ability to travel locally up to 75% of the time
  • Access to a designated quiet workspace in the home
  • Reside in an area with a reliable high-speed internet connection
  • Access to reliable transportation & valid US driver's license

Preferred Qualifications

  • Bachelor’s Degree (or higher) in a health-related field
  • Active Licensed Practical Nurse (LPN) in Idaho, Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant
  • 2+ years of field-based experience
  • Experience working in Managed Care
  • Previous care management experience
  • CHR/CHW Accreditation
  • Familiarity with resources available in Tribal communities
  • Knowledge of culture, sovereignty, and values of the Tribal community
  • Knowledge and Training on SDOH (Social Determinants of Health)
  • Knowledge of Medicaid and/or Medicare population
  • Speak one or more Idaho Tribal languages
