Your role will vary from conducting assessments, developing individualized care plans, administering medication, and collaborating with a team of healthcare professionals to support patient health and independence. The typical day includes visiting 5-7 patients, managing case documentation, and utilizing advanced EMRs to streamline patient care processes.
