RN Contract - Case Manager Auto-Offer
Position: Travel Registered Nurse - Case Manager
Facility: Providence Santa Rosa Memorial Hospital
Location: Santa Rosa, California
Contract Length: 13 weeks
Shift: 5x8s Days (8 AM-4:30 PM)
Setting: Acute Care Case Management
Must Have:
️ 2+ years recent acute hospital case management experience (care coordination & discharge planning in a fast-paced environment)
️ Travel RN experience
️ BLS certification
️ Epic charting experience (preferred)
️ Comfort managing a 1:20 patient ratio
️ Current work history within the last 2 years

You'll Be Doing:
Care coordination & discharge planning
Arranging DME, SNF placement & ambulance transport independently
Collaborating in interdisciplinary meetings
Working with HMOs, IPAs & insurance plans
