Key Responsibilities
Conduct and complete the Minimum Data Set (MDS) assessment
Evaluate residents’ conditions and medical data
Prepare and electronically transmit reports to Medicare and Medicaid
Develop a written plan of care for each resident
Provide information on Medicare/Medicaid and financial assistance programs
Ensure MDS notes are descriptive of services provided
Assist with care plan portion of residents’ discharge plan
Required Qualifications
Current, valid state RN license
Three to five years’ experience in an MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment
Current, valid CPR certification
