Job Description
Join Centene as a Licensed Practical Nurse (LPN) and play a crucial role in transforming healthcare for 28 million members. This position focuses on developing personalized care management strategies for mental health and substance use disorders, providing high-quality care and support to members and their families.
Key Responsibilities
Develop and facilitate care management activities for mental and behavioral health needs.
Evaluate member needs and recommend care plans.
Conduct telephonic, digital, and in-home visits.
Coordinate care between members, families, and providers.
Monitor care plans for changes in treatment and outcomes.
Educate members and families on healthcare options.
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2 – 4 years of related experience
LCSW or LMHC certification
Preferred Qualifications
Experience in pediatric care
Knowledge of mental health and substance use disorders
