Job Overview

  • Provides skilled nursing and case management for patients in home/residential settings
  • May start as a Visiting RN and transition to Case Manager; clinical responsibilities are the same
  • Works collaboratively within an interdisciplinary team to ensure continuity and quality of care
  • Coordinates resources, services, and patient/family education to ensure optimal clinical and cost outcomes

Key Responsibilities

  • Deliver direct patient care and skilled nursing interventions per physician orders
  • Assess and monitor physical and psychosocial conditions, adjust plans of care accordingly
  • Supervise and evaluate Home Health Aides and CNAs
  • Ensure continuity of care through effective communication with other care providers and support agencies
  • Provide patient/family education, counseling, symptom management, and referrals
  • Maintain accurate and compliant documentation, including OASIS requirements
  • Participate in team scheduling, on-call rotation, and respond to care needs via phone or home visits
  • Engage in quality improvement activities and attend required in-service trainings
  • Perform additional duties as assigned

Qualifications

  • Current Montana RN license – Required (BSN preferred)
  • Valid driver’s license, reliable transportation, and appropriate auto insurance – Required
  • 2+ years recent experience in acute care, home care, or hospice – Preferred
  • Clinical skills preferred:
    • Venipuncture
    • Catheterization
    • Wound care
    • Parenteral therapy
