Job Description
Join The Jewish Board as a Case Manager, where you'll support adults with serious mental illness in achieving independence through personalized recovery goals. This role involves both direct resident support and administrative coordination, ensuring quality care and advocacy for vulnerable individuals in a compassionate environment.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning with residents
Ensures service plans are completed and signed by residents
Evaluates and assesses independent living skills
Supports residents towards medication management independence
Advocates on behalf of residents with other service providers
Completes regular documentation of all resident interactions
Links residents to community-based treatment
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
