Key Responsibilities
Conduct initial and periodic in-person assessments to evaluate participants’ functional abilities and rehabilitation needs
Recommend and coordinate adaptive equipment and home modifications
Develop, implement, and revise individualized treatment plans
Deliver OT interventions across various settings
Educate participants and caregivers on equipment use and home safety strategies
Maintain documentation of assessments, treatment, and participant progress
Participate in daily interdisciplinary team meetings
Support quality improvement and compliance initiatives
Required Qualifications
Advanced degree in Occupational Therapy from an accredited school
Current license to practice Occupational Therapy in the state
Two years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting
Preferred Qualifications
One year of experience working with a frail or elderly population
