Job Description
Join The Jewish Board as a Senior Case Manager, where you'll support adults with serious mental illness in achieving independence through personalized recovery goals. You'll lead complex caseloads and advocate for residents, enhancing their quality of life in a supportive community environment.
Key Responsibilities
Carries a complex caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Supports residents in medication management independence
Advocates for residents with other service providers
Completes regular documentation of resident interactions
Participates in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
In-depth understanding of how mental illness, substance use disorders, or trauma can affect individuals
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Knowledge of local and state health and human service systems
Preferred Qualifications
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
