Job Description

Join The Jewish Board as a Senior Case Manager, where you'll support adults with serious mental illness in achieving independence. You'll manage complex cases, advocate for residents, and ensure effective service planning in a compassionate and inclusive environment.

Key Responsibilities

  • Carries a caseload and practices person-centered service planning
  • Evaluates and assesses independent living skills
  • Advocates for residents with service providers
  • Completes regular documentation of resident interactions
  • Participates in staff meetings and trainings

Required Qualifications

  • Bachelor of Arts (BA) or Bachelor of Science (BS) degree
  • At least 1-year experience in Human Services preferred
  • In-depth understanding of mental illness and trauma
  • Experience working with individuals with serious mental illness
  • Knowledge of local and state health and human service systems

Preferred Qualifications

  • Familiarity with Microsoft Office suite
  • Valid driver's license and willingness to drive agency van
