Key Responsibilities
Develop and implement care plan interventions as a single point of contact
Communicate required health-related information to stakeholders
Advocate for patients to ensure their needs are met
Assess, plan, and implement individualized care strategies
Identify problems/barriers to care and provide management interventions
Initiate referrals for social service programs
Provide resource support to members for local services
Manage the person-centered service/support plan
Conduct home visits and in-person visits to various facilities
Maintain compliance with state and federal guidelines
Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or Master's/Bachelor’s degree with relevant social work licensure
2+ years of experience in community health settings
1+ years of experience with local behavioral health providers
1+ years of experience working with individuals with long-term care needs
1+ year experience with electronic documentation systems and MS Office
Access to reliable transportation and ability to travel within assigned territory
Access to a designated quiet workspace at home
Ability to travel to Southfield, MI for quarterly meetings
Must reside in Michigan
Preferred Qualifications
RN or LMSW, LLMSW, LCSW
1+ years of medical case management experience
Experience in Motivational Interviewing, Trauma-Informed Care
Experience serving individuals with co-occurring disorders
Experience with MI Health Link
Experience working in Managed Care
Knowledge of NCQA documentation standards
