Key Responsibilities

  • Develop and implement care plan interventions as a single point of contact
  • Communicate required health-related information to stakeholders
  • Advocate for patients to ensure their needs are met
  • Assess, plan, and implement individualized care strategies
  • Identify problems/barriers to care and provide management interventions
  • Initiate referrals for social service programs
  • Provide resource support to members for local services
  • Manage the person-centered service/support plan
  • Conduct home visits and in-person visits to various facilities
  • Maintain compliance with state and federal guidelines

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or Master's/Bachelor’s degree with relevant social work licensure
  • 2+ years of experience in community health settings
  • 1+ years of experience with local behavioral health providers
  • 1+ years of experience working with individuals with long-term care needs
  • 1+ year experience with electronic documentation systems and MS Office
  • Access to reliable transportation and ability to travel within assigned territory
  • Access to a designated quiet workspace at home
  • Ability to travel to Southfield, MI for quarterly meetings
  • Must reside in Michigan

Preferred Qualifications

  • RN or LMSW, LLMSW, LCSW
  • 1+ years of medical case management experience
  • Experience in Motivational Interviewing, Trauma-Informed Care
  • Experience serving individuals with co-occurring disorders
  • Experience with MI Health Link
  • Experience working in Managed Care
  • Knowledge of NCQA documentation standards
