Join Molina Healthcare as an IRIS Consultant, where you'll make a meaningful impact by helping individuals with long-term care needs navigate the Wisconsin IRIS program. This remote role focuses on building relationships and supporting participants in achieving their personal goals through customized plans and community resources.
Key Responsibilities
Meet in person with IRIS participants a minimum of four times per year
Provide program orientation to new participants
Assist participants in identifying personal outcomes
Document all activities within the IRIS data system
Research community resources for participants
Educate participants on budget management
Develop trusting relationships with participants
Attend monthly team meetings and weekly check-ins
Required Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related field
1+ year of direct experience in delivering social services
Ability to work independently and be self-motivated
Knowledge of Long-Term Care programs
Excellent problem-solving and critical thinking skills
Strong time management and prioritization skills
Ability to develop and maintain professional relationships
Comfortable working with diverse populations
Resourceful with knowledge of community resources
Proficient in various technologies including Outlook, Skype, Teams, PowerPoint, Excel, Word
