Key Responsibilities

  • Develop and implement care plan interventions as a single point of contact
  • Communicate with stakeholders to ensure quality coordinated care
  • Advocate for patients and families to support their needs and choices
  • Assess and implement individualized care strategies
  • Identify barriers to care and provide management interventions
  • Initiate referrals for social service programs
  • Provide resource support for local services
  • Manage the person-centered service/support plan
  • Conduct home and in-person visits
  • Document and maintain member information to ensure compliance

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or a Master's or Bachelor's degree in Social Work with relevant licensure
  • 2+ years of experience in a community health setting
  • 1+ years of experience with local behavioral health providers and community support organizations
  • 1+ years of experience working with long-term care needs
  • 1+ years of experience in electronic documentation systems and with MS Office
  • Access to reliable transportation and the ability to travel 75% of the time
  • Access to a designated quiet workspace in your home
  • Ability to travel to Southfield, MI office for quarterly meetings
  • Must reside within the state of Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Experience or training in Motivational Interviewing, Trauma-Informed Care, Person-Centered Care
  • Experience serving individuals with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
