Key Responsibilities

  • Conduct comprehensive bio-psychosocial assessments and establish care plans
  • Collaborate with interdisciplinary teams to assess care levels and develop care/discharge plans
  • Utilize therapeutic methods in accordance with care plans
  • Monitor progress and revise plans to meet patient needs
  • Provide education to patients and families
  • Provide direct patient care including case management, crisis intervention, and counseling
  • Maintain patient-centered documentation
  • Participate in administrative activities and clinical supervision
  • Support the development of unit-based programming
  • Contribute to improved patient experience

Required Qualifications

  • Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or Related Human Service Field
  • Previous experience in a healthcare setting
  • LCSW, LPC, LMFT, LADC license
  • Valid Driver's License
  • Computer proficient
  • Ability to assess problems and make decisions in a timely manner
  • Excellent communication skills
  • Focused on personal, patient/client, and unit/staff safety

  • Demonstrates competencies to meet age-specific patient needs
