Join Cooper University Health Care as a Patient Navigator, where you'll play a crucial role in bridging the gap between patients and clinicians. Your support will help patients navigate the health care system, ensuring continuity of care while collaborating with a dedicated team to enhance patient experiences.
Key Responsibilities
Bridge the gap between clinician and patient by providing support in navigating the health care system
Assist with scheduling, pre-certification processes, follow-up calls, and record retrieval
Call new patients within 24 hours of their visit for follow-up
Work collaboratively with various healthcare teams to support patient needs
Participate in daily multidisciplinary rounding on the inpatient oncology unit
Assist patients in transitioning from inpatient to outpatient areas
Connect patients to additional services for support
Track patient contacts, follow-ups, and barriers to care
Ensure delivery of patient care through coordination of services
Required Qualifications
Experience working in a healthcare office setting or community outreach and education
Knowledge of health care, inpatient/outpatient settings, Oncology specific patient population
Strong interpersonal skills
Ability to take direction and work independently
Flexibility, optimism, and a cooperative working style
Preferred Qualifications
Clinical training (i.e. MA, CNA, LPN)
Bilingual Spanish/English
Competent in basic computer skills
Knowledge of using an electronic health records system
