Job Description

Join Cooper University Health Care as a Patient Navigator, where you'll play a crucial role in bridging the gap between patients and clinicians. Your support will help patients navigate the health care system, ensuring continuity of care while collaborating with a dedicated team to enhance patient experiences.

Key Responsibilities

  • Bridge the gap between clinician and patient by providing support in navigating the health care system
  • Assist with scheduling, pre-certification processes, follow-up calls, and record retrieval
  • Call new patients within 24 hours of their visit for follow-up
  • Work collaboratively with various healthcare teams to support patient needs
  • Participate in daily multidisciplinary rounding on the inpatient oncology unit
  • Assist patients in transitioning from inpatient to outpatient areas
  • Connect patients to additional services for support
  • Track patient contacts, follow-ups, and barriers to care
  • Ensure delivery of patient care through coordination of services

Required Qualifications

  • Experience working in a healthcare office setting or community outreach and education
  • Knowledge of health care, inpatient/outpatient settings, Oncology specific patient population
  • Strong interpersonal skills
  • Ability to take direction and work independently
  • Flexibility, optimism, and a cooperative working style

Preferred Qualifications

  • Clinical training (i.e. MA, CNA, LPN)
  • Bilingual Spanish/English
  • Competent in basic computer skills
  • Knowledge of using an electronic health records system
