Job Description
Join BoldAge PACE as a PCP Nurse Practitioner and play a vital role in enhancing the quality of life for seniors. You'll provide comprehensive care tailored to meet the individual health needs of older adults, working in a collaborative team environment.
Key Responsibilities
Provide direct and indirect care to participants
Perform health histories, physical exams, and participant teaching
Order, interpret, and follow up on lab tests
Manage complex clinical situations
Document care in EMR and initiate emergency measures
Coordinate participant care under supervision of attending physician
Act as liaison with healthcare professionals
Participate in interdisciplinary team meetings and care planning
Evaluate and treat participants in various settings
Required Qualifications
Master’s degree in clinical nursing from accredited Nurse practitioner program
3-5 years nursing experience with Nurse Practitioner experience preferred
RN licensure in perspective state
Family certification from Nurse Practitioner program
1 year of experience working with a frail or elderly population preferred
Preferred Qualifications
Training on working with a frail or elderly population will be provided upon hiring
Reliable transportation and valid driver's license
Medically cleared for communicable diseases and up-to-date immunizations
