Job Description
Join The Jewish Board as a Case Manager, where you'll support adults with serious mental illness in achieving independence and recovery. You'll work closely with residents, advocating for their needs and facilitating their access to essential services.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning with residents
Supports residents in meeting individual goals and medication management independence
Advocates on behalf of residents with other service providers
Completes documentation of all resident interactions
Links residents to community-based treatment and collaborates with other service providers
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
