Job Description:
We’re seeking a compassionate and experienced Hospice RN Case Manager to provide exceptional end-of-life care in a home health setting. In this role, you will admit and assess patients, develop and implement individualized care plans, and coordinate with a multidisciplinary team to ensure each patient’s physical, psychosocial, and emotional needs are met. The ideal candidate will have at least one year of hospice nursing experience, strong communication skills, and the ability to provide empathetic, patient-centered care.

Responsibilities:

• Perform comprehensive assessments and develop personalized care plans.
• Coordinate care with physicians, social workers, and other members of the hospice team.
• Provide patient and family education on end-of-life care.
• Document all patient interactions and interventions in accordance with policy and compliance standards.
• Participate in quality improvement and interdisciplinary team meetings.
• Conduct follow-up visits and ensure continuity of care.

Schedule:

• Day shifts: 8:30 AM – 5:00 PM (flexible based on patient needs)
• One weekend per month may be required

Territory Coverage:
Serving patients across Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose, Saratoga, Los Gatos, Campbell, Morgan Hill, Gilroy, and nearby areas.

Requirements:

• Active RN license (state of California)
• Minimum 1 year of hospice nursing experience
• Current BLS certification
• Proficiency with Homecare Homebase (HCHB) preferred
