Key Responsibilities
Deliver high-quality care as a member of the PACE interdisciplinary team
Provide personal care assistance including bathing, dressing, grooming, feeding, and toileting
Facilitate and participate in group and one-on-one activities
Assist with meal preparation, service, monitoring, and cleanup
Maintain a clean, safe, and orderly environment for participants
Record and monitor vital signs and changes in participants’ health
Accompany participants to appointments and provide escort services
Collaborate with the IDT to develop and implement individualized care plans
Maintain accurate documentation in participant medical records
Foster positive relationships with participants, families, and co-workers
Participate in team meetings and training for quality improvement
Required Qualifications
Two years of experience in a hospital, nursing home, or community-based setting
License/Certification as applicable per State requirements
1 year of experience working with a frail or elderly population preferred
Preferred Qualifications
Training will be provided upon hiring if applicable for the role
