Key Responsibilities
Deliver and coordinate quality patient care in compliance with physician orders
Continuously observe and assess patient condition and care needs
Document all activities, assessments, and nursing actions in a timely manner
Participate in the implementation and updating of the nursing care plan
Educate patients and families regarding disease processes and self-care techniques
Required Qualifications
Active, unencumbered LPN/LVN license in Texas
Current CPR certification
TB skin test (current within last 12 months)
Reliable transportation
Preferred Qualifications
Six months prior hands-on nursing experience
Home health experience
