Key Responsibilities
Serve as a case planner to ensure client care
Develop action plans utilizing existing resources
Conduct community contacts and evaluations
Run groups under supervision of a social worker
Evaluate clients for individualized treatment programs
Provide individual/group counseling as needed
Perform home visits to assess family and child safety
Facilitate client referrals to service providers
Conduct community outreach and establish linkages
Maintain necessary documentation and progress notes
Required Qualifications
BA Degree in a related field
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
