As a Care Coordinator at The Jewish Board, you'll play a critical role in helping individuals with chronic behavioral health and medical conditions access essential services. You'll work collaboratively to develop care plans, manage services, and support members in achieving optimal health outcomes.
Key Responsibilities
Link adults and children with chronic behavioral health and medical conditions to necessary services
Assess risk and needs, develop person-centered care plans
Provide care management services and track appointments
Educate members and coordinate health and community services
Conduct periodic assessments of members' health needs
Provide outreach services to members
Required Qualifications
Bachelor's degree in social work, psychology, nursing, or related field
Two years of experience in direct services or case management for mentally disabled, chronically ill, or homeless individuals
Experience in interdisciplinary teams
Experience in care management or coordination in a medical or behavioral health environment
Preferred Qualifications
Fluency in a second language such as Spanish, Russian, or Creole
