Job Description

Join Primary Vascular Care as a Nurse Practitioner to deliver on-site clinical assessments in a mobile practice for long-term care residents. You'll play a crucial role in patient treatment planning and maintaining high-quality care while working independently in a supportive environment.

Key Responsibilities

  • Perform on-site screening clinical assessments to determine appropriate patient treatment plans
  • Function independently to perform history and physical assessments for complex patients
  • Order and review diagnostic and therapeutic tests
  • Assist with the provision of care in accordance with regulations
  • Monitor the effectiveness of interventions
  • Maintain relationships with contacts in facilities
  • Educate family members and clients about services and programs
  • Assist with the development of physician relations
  • Complete reports in a timely manner

Required Qualifications

  • Bachelor's Degree
  • Degree from an accredited Nurse Practitioner or Physician Assistant Program
  • Current NP License
  • BLS - Basic Life Support through American Heart Association
  • Current Infection Control Certification
  • Board Certified/Board Eligible
  • Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella

Preferred Qualifications

  • ACLS - Advanced Cardiac Life Support through American Heart Association
  • COVID and Flu Vaccine
  • 1 year of NP experience
