Key Responsibilities
Deliver skilled nursing services in-home including wound care, medication administration, and post-surgical care
Work 1:1 with patients, building trust and rapport
Collaborate closely with care team and patient's physician
Keep clear, accurate documentation and comply with DOH standards
Required Qualifications
Licensed Practical Nurse (LPN) licensed in Pennsylvania
CPR-certified
Able to travel locally with own vehicle
Skilled communicator with a patient-centered approach
