Job Description
Join The Jewish Board as a Case Manager, where you'll empower adults with serious mental illness to live independently in their communities. Your role will involve personalized support, advocacy, and collaborative service planning to help residents achieve their recovery goals in a culturally competent and compassionate environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning with residents
Ensures service plans are completed according to regulatory guidelines
Supports residents towards medication management independence
Advocates for residents with other service providers
Completes regular and timely documentation of resident interactions
Links residents to community-based treatment
Participates in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
