Job Description

Key Responsibilities
Prepares weekly reports for management regarding patient information
Oversees membership reports and updates patient data bi-weekly
Coordinates initial patient contact and verifies eligibility
Prepares and sends new patient packets
Initiates Comfort Care appointments as needed
Completes monthly membership reports
Coordinates with customer service for unreachable patients
Validates patients' attestation sheets
Ensures timely submission of attestation sheets
Tracks the Data Rap process for timely scheduling
Provides customer service to patients in person and via phone
Inputs and updates patient demographic information
Schedules routine and follow-up appointments
Required Qualifications
High School Diploma/GED (or higher)
3+ years of experience working in a medical office
Intermediate level of proficiency in PC software (Microsoft Word, Excel, Power Point)
Ability to commute 25% of the time
Access to reliable transportation and valid US driver’s license
Preferred Qualifications
Knowledge of medical terminology and anatomy
Planning, organizing, conflict resolution, negotiating and interpersonal skills
Independent problem identification/resolution and decision-making skills
Ability to prioritize, plan, and handle multiple tasks
Strong communication and inter-personal skills
Excellent written and verbal communication skills
Bilingual in English & Spanish
