Join Molina Healthcare as a Field Family Nurse Practitioner, where you'll provide essential preventive care in the comfort of patients' homes and communities. You'll perform comprehensive medical assessments, coordinate care, and collaborate with fellow practitioners while addressing healthcare gaps in underserved populations.
Key Responsibilities
Provide general medical care and care coordination
Perform comprehensive evaluations and establish medical diagnoses
Order diagnostic tests and perform necessary medical procedures
Create and implement a medical plan of care
Coordinate post-discharge care to reduce hospital readmissions
Participate in community-based clinics
Drive to various locations and maintain cross-state licenses
Document medical records in an electronic system
Required Qualifications
Master's degree in family health from accredited nursing program
Active and unrestricted national certification from a recognized organization
Current state-issued license to practice as a Family Nurse Practitioner
Current Basic Life Support for Healthcare Professional certification
Current unrestricted driver’s license
Advanced computer skills, proficient with Word, Excel, and Electronic Medical Record
Preferred Qualifications
3-5 years of experience as a Registered Nurse and/or Nurse Practitioner
Experience in home health, community health, or public health setting
Experience with underserved populations
Fluency in a language in addition to English
Immunization and point of care testing skills
