Overview:

The RN II Virtual Primary Care provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advance practitioners, the RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits in between regular office visits, assisting in panel manage

Qualifications:

• Education: Graduation from an accredited registered nursing program. Bachelor’s degree in nursing (BSN) or equivalent work experience.
• Experience: Minimum 5 years healthcare experience required.
