The Case Manager will provide community-based services to individuals identified as eligible for behavioral health services, including assessing and monitoring resources that help the client live in the community in a stable and safe manner Schedule Details:

$21.00/hour Job Functions:
Carry a caseload of up to 30 clientsPerform case management duties for consumers involved in Behavioral Health ServicesProvide services based on Recovery Model principlesDevelop individualized, strength-based service plans that are measurable and lead toward targeted outcomesAssists consumer service needs and assists consumers in assessing appropriate mental health servicesAssist consumers in obtaining and maintaining basic needs such as housing, food, healthcare, employment, and socializationAssist consumers through assertive and creative efforts to gain needed resources and services identified in the service planMonitor the consumer's participation in the recovery plan and support servicesProvide effective crisis assessment and crisis intervention to consumers when necessaryMaintain professional relations with the consumer, their families, payers, community support service representatives, coworkers, and other agenciesEnsure appropriate communication and coordination of effort between all the consumer's service providers and support systemsAct as an effective "single point of contact" for multiple health and social services linkagesProvide all authorized client service and provide supporting documentation for re-authorizations, as necessary, in a timely mannerSubmit accurate and timely payroll and billing documentationNeed to be available for an on-call rotation 24/7Minimum Requirements:
Education/Experience/Licensure (Must Meet One of the Following Criteria):
A bachelor's degree with major coursework in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or educationBe a registered nurseA high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science, and 2 years experience in public or private human services with 2 years in direct client contact ( 1 year for Lehigh and Northampton counties )A high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.Clearances:
Pennsylvania Child Abuse, Criminal and fingerprint-based federal criminal history; Verification that employee is not on any Medicaid/Medicare Exclusion list Note:
