Key Responsibilities
Provide on-call physical therapy evaluations and care plans
Collaborate with interdisciplinary teams and external caregivers
Develop and implement the PT plan of care
Communicate and coordinate care with team and outside providers
Function independently in multiple settings
Required Qualifications
Bachelor's Degree in Physical Therapy
National Provider BLS - American Heart Association within 30 days of hire
Washington Physical Therapist License upon hire
Preferred Qualifications
Master's Degree in Physical Therapy
D.P.T. in Physical Therapy
1 year working with a geriatric population
