Job Description
Join The Jewish Board as a Case Associate, where you will make a significant impact in the lives of clients by developing action plans and connecting them with essential resources. This role involves community outreach, case management, and providing support for independent living skills, all while being part of an organization that values diversity and inclusivity.
Key Responsibilities
Function as a case planner and supply appropriate interventions for client care
Develop action plans utilizing existing and new resources for clients
Perform routine community contacts and home visits
Evaluate living conditions and assess independent living skills of clients
Facilitate client referrals to treatment or service providers
Provide individual/group counseling
Conduct community outreach and establish linkages with other organizations
Maintain necessary documentation and progress notes
Required Qualifications
BA Degree
Experience in a community or social service group
Five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
