Job Summary

We are seeking a dedicated Occupational Therapist to join a Skilled Nursing Facility on a 13-week contract assignment. The OT will work with residents to improve functional abilities, support rehabilitation goals, and promote independence through individualized treatment plans. This role offers a guaranteed weekly hour schedule and requires flexibility to work varying shifts.

Key Responsibilities

  • Conduct evaluations to assess residents’ functional status, mobility, and safety needs.
  • Develop and implement patient-centered treatment plans based on clinical findings.
  • Provide therapeutic interventions to support recovery, strength, mobility, and independence.
  • Collaborate with an interdisciplinary care team to optimize patient outcomes.
  • Document all assessments, progress notes, and care plans accurately and in a timely manner.
  • Educate residents and caregivers on safe techniques, adaptive equipment, and ongoing home-care recommendations.
  • Adhere to facility policies, professional standards, and state regulatory requirements.

Qualifications

  • Active and current state Occupational Therapy license (required).
  • Minimum of 1 year of experience in a rehabilitation or SNF setting.
  • Strong communication, assessment, and clinical decision-making skills.
  • Ability to work independently and adapt to varying shift schedules.
  • Commitment to delivering high-quality, patient-focused care.
