???? Key Responsibilities

• Deliver high-quality nursing care to residents in long-term care and skilled nursing facilities.
• Assess patient conditions and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor vital signs and report changes to healthcare providers.
• Collaborate with interdisciplinary teams to ensure coordinated and effective care.
• Educate patients and families on health management and chronic condition care.

✅ Requirements

• Active RN license in the state of Rhode Island.
• Minimum 2 years of LTC/SNF experience preferred.
• Strong clinical assessment and documentation skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-focused approach.
