As a Speech Language Pathologist at CenterWell Home Health, you will provide personalized therapy services focusing on speech, language, hearing, and swallowing disorders. You will assess and treat patients in their homes, collaborating with healthcare teams to support patient rehabilitation and enhance their quality of life.
Key Responsibilities
Evaluate, direct and provide speech/language pathology service to patients in the home or facility
Participate in the development and periodic review of the Plan of Treatment and Plan of Care
Utilize professional skills and judgment in assessing and treating speech, voice, language, hearing and swallowing disorders
Administer and interpret diagnostic tests and therapeutic treatments
Observe, record and report changes in the patient's condition and response to treatment
Provide instruction and training to patients in alternative communication systems
Counsel and instruct patients, families and healthcare staff
Maintain and submit required documentation
Participate in care coordination activities and discharge planning
Maintain professional conduct regarding confidential information
Attend and participate in internal staff development programs
Required Qualifications
Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
Minimum of six months experience as a speech therapist / speech language pathologist
Current and unrestricted license
Current CPR certification
Good organizational and communication skills
Valid driver’s license, auto insurance, and reliable transportation
