**Job Description:**

We are seeking a Home Health Physical Therapist to provide top•notch care to patients in their homes. This role demands expertise in home health services and proficiency in HCHB/OASIS documentation.

**Responsibilities:**

• Deliver quality physical therapy to home health patients.
• Complete same•day documentation using HCHB.
• Manage a weekly caseload of 20•25 cases, achieving 26•28 productivity points.
• Attend weekly in•person office meetings.
• Be flexible to float and meet weekend requirements.

**Qualifications:**

• Valid PT License and BLS Certification.
• 2+ years of home health experience.
• Proficient in HCHB/OASIS, with Oasis E experience post•January 2023.
• Efficient caseload management skills.

**Additional Information:**

• 13•week contract, 40•hour workweek.
• No on•call duties.
• Time off requires prior approval and must be made up.
• Scrub color: Any.
• Orientation includes 2•3 days of modules and team introduction.

**Note:** Candidates must live within 30 minutes of the territory or arrange nearby housing if traveling.
