Title: Physical Therapist (Home Health) – Mesa, AZ

Location: Mesa, AZ

Job Type: Full-Time (Monday‑Friday shifts possible for Home Health)

Responsibilities:

• Perform patient evaluations, develop and implement evidence-based treatment plans
• Collaborate with patients and caregivers to set functional, measurable goals
• Document care accurately and within departmental timelines
• Communicate patient progress/results to multidisciplinary team and referring physicians
• Supervise PT assistants, therapy technicians, and students where applicable
• Ensure compliance with Banner Health policies and regulatory requirements

JPC - 20186 Minimum Qualifications: Graduate of an APTA‑accredited PT program Active Arizona PT license or valid PT Compact privilege including AZ BLS/CPR certification (must be current) AZ Fingerprint Clearance Card Ability to use clinic EMR and documentation tools effectively
