Job Description
Join Johns Hopkins Medicine as an Oncology Social Worker, where you will provide impactful counseling and support services to patients and families navigating cancer care. This role allows you to collaborate with healthcare professionals and facilitate resources to enhance patient well-being and care transitions.
Key Responsibilities
Provide counseling to individuals and families throughout the cancer experience
Identify psychosocial needs of cancer patients/families
Facilitate a general cancer support group
Develop discharge plans for patients with complex needs
Act as a consultant to Case Coordinator for in-patients
Document activities in the medical record
Coordinate transportation logistics for patients
Liaison with external providers to arrange post-treatment services
Required Qualifications
Licensed to practice as a Social Worker in the District of Columbia
Masters degree in Social Work from an accredited school
Minimum of 2-5 years social work experience in a health care setting
Current CPR Certification
