Job Description

Join Christian Living Communities as an Entry-Level MDS Coordinator, where you'll collaborate with multiple departments to develop and manage resident assessments and care plans. This role supports a warm environment for residents while offering opportunities for training and professional growth.

Key Responsibilities

  • Oversee and evaluate assessments and care plans
  • Monitor completion of PPS, OBRA assessments, and CAAs
  • Maintain and update policies for resident assessment and care plans
  • Transmit completed MDS weekly; maintain records for surveyors
  • Assess prospective residents with team members
  • Perform nursing functions as needed
  • Complete administrative duties (forms, reports, evaluations)
  • Supervise hiring, performance management, and evaluations
  • Take on-call status as assigned
  • Develop quality assurance programs for assessments and care plans
  • Participate in on-site surveys by government agencies
  • Other duties as assigned

Required Qualifications

  • Colorado State Licensed Practical Nurse license, in good standing
  • Minimum of two years of managerial experience in a hospital or nursing care
  • Knowledge and skills for age-related care needs
  • Schedule flexibility
  • Problem-solving abilities
  • Knowledge of medical practices and procedures
  • Basic knowledge of office software (Word, Outlook, Excel)
  • Fluent in English
  • Excellent customer service skills
