Key Responsibilities
Conduct PT evaluations and develop PT plans of care
Coordinate care with the Interdisciplinary Team and external providers
Work in various settings including PACE centers, participant homes, and nursing facilities
Assume responsibility for participants treated by the PTA
Required Qualifications
Bachelor's Degree in Physical Therapy
National Provider BLS - American Heart Association within 30 days of hire
Washington Physical Therapist License upon hire
Preferred Qualifications
Master's Degree in Physical Therapy
D.P.T. in Physical Therapy
1 year working with a geriatric population
