**Job Title: Home Health Occupational Therapist • Days**

**Location:** San Ramon, CA

**Job Description:**

We are seeking a Home Health Occupational Therapist to provide patient care in their homes. This role requires expertise in home health and proficiency in HCHB/OASIS documentation.

**Key Responsibilities:**

• Provide occupational therapy to home health patients.
• Document using Homecare Homebase (HCHB) on•site.
• Manage 20•25 cases weekly, achieving 5•7 productivity points daily.
• Attend weekly in•person office meetings.
• Be flexible to float as needed.

**Requirements:**

• Valid California Occupational Therapist License.
• Basic Life Support (BLS) certification.
• 2+ years of recent home health experience.
• Proficiency in HCHB/OASIS, with recent experience.
• Experience with Oasis E post•January 2023.
• Ability to work 40 hours weekly, including potential weekends.

**Additional Information:**

• 2•3 days of orientation and training.
