Job Description
Join Loyal Home Care Services as a Licensed Practical Nurse (LPN) where you will deliver personalized care and support to clients in Franklin County, MA. Enjoy a flexible schedule while making a meaningful impact in the lives of patients and their families.
Key Responsibilities
Deliver skilled nursing care based on the Plan of Care
Support and carry out care plans initiated by the Registered Nurse
Provide education and guidance to patients and families on condition management
Monitor health status, document accurately, and report updates
Collaborate with RNs and coordinate additional services
Use medical equipment safely, maintaining infection control standards
Stay current on best practices through ongoing clinical development
Be an advocate and source of encouragement for patients
Required Qualifications
Active LPN license in Massachusetts
Graduate of an accredited LPN or vocational nursing program
Minimum of 3 years nursing experience (home health or med/surg preferred)
Current CPR/BLS certification
Reliable transportation and valid driver's license
Up-to-date health certificate & TB test
Fluent in English (spoken and written)
Demonstrated knowledge of HIPAA & patient confidentiality
Must be 18 years or older and authorized to work in the U.S.
