Position Summary:

We are seeking a compassionate and skilled Registered Nurse (RN) to join our hospice care team. The RN will provide expert clinical care, emotional support, and education to patients with terminal illnesses and their families, promoting comfort and quality of life in accordance with hospice philosophy.

Key Responsibilities:

• Conduct comprehensive nursing assessments and develop individualized care plans.
• Manage symptoms and provide palliative care to ensure patient comfort.
• Administer medications and treatments as prescribed.
• Collaborate with interdisciplinary team members including physicians, social workers, chaplains, and aides.
• Provide education and support to patients and families regarding disease progression, symptom management, and end-of-life planning.
• Maintain accurate and timely documentation in compliance with regulatory standards.
• Participate in interdisciplinary team meetings and family conferences.
• Respond to patient and family concerns with empathy and professionalism.
• Coordinate care with other healthcare providers and community resources.
