Job Description
Join UVM Home Health & Hospice as a compassionate LPN to provide vital nursing care in patients' homes. Make a meaningful impact through comprehensive patient support and collaboration with a dedicated healthcare team.
Key Responsibilities
Provide skilled nursing care under the supervision of an RN, following established plans of care
Assess patients' physical, emotional, and social needs
Administer medications, wound care, and other treatments
Educate patients and families on healthcare needs and self-management strategies
Collaborate with other healthcare professionals to ensure comprehensive care
Required Qualifications
Active LPN license in Vermont
Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience
Strong assessment, communication, and interpersonal skills
Compassionate and patient-centered approach
Excellent teamwork and collaboration abilities
