As an IRIS Consultant, you will build lasting relationships and help individuals navigate the Wisconsin IRIS program, supporting their long-term care goals and connecting them with community resources. This remote position allows for flexibility while ensuring impactful engagement with clients in their homes and communities.
Key Responsibilities
Meet in person with IRIS participants a minimum of four times per year
Provide program orientation to new participants
Explore participants' lives and identify personal outcomes
Document all activities in the IRIS data system
Research community resources for participants
Educate participants on budget reports
Maintain confidentiality and HIPAA compliance
Collaborate with other IC staff and attend team meetings
Required Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related field
1+ years of direct experience in social services
Ability to work independently and self-motivated
Knowledge of Long-Term Care programs
Excellent problem-solving and critical thinking skills
Strong time management and prioritization skills
Ability to maintain professional relationships
Comfortable working with diverse populations
Resourceful with knowledge of community resources
Proficient in technology including Outlook, Skype, Teams, PowerPoint, Excel, Word
